Our Medicaid Service Coordination program pairs developmentally disabled children and adults with trained and experienced service coordinators who develop a tailored plan to gain access to the necessary services in the community that meet the needs of each individual. Our talented service coordinators meet each individual in his or her home, as well as in the community, to provide advocacy, assistance and support. The Service Coordinator can arrange for services from an array of government, community and private agencies and organizations in addition to family education and training. This program is available to any individual with a developmental disability residing in Westchester County who meets eligibility requirements. Our Medicaid Service Coordinators: Advocate for services, Help obtain and maintain health and government benefits such as; Medicaid, Social Security and Food Stamps, Provide information and referrals for: Counseling, Program and Services Enrollment, Camp and Recreational Activities, Respite, Guardianship and Residential Services, Help to reduce the time it takes to develop contacts with community services such as doctors, equipment specialists and vendor, and Assist in the completion of administrative procedures (applications, paperwork for enrollment for benefits, etc.).
